Case Management Support
and Short-Term Housing Help
Case management is the primary tool used to successfully prevent a family from becoming homeless or entering emergency shelter. Every family receives one-on-one case management services including: individualized housing stabilization plans; budget counseling; landlord/family negotiations; basic tenancy skills; job coaching and search; connection to community resources; and, when necessary, short-term financial assistance to preserve existing housing situations or create new ones (security deposits, past due rents, rental subsidies, and utility arrearages).
In FY 2020, Always Home served 326 client families seeking emergency housing assistance. By focusing on effective homelessness prevention strategies, 322 families were successfully diverted from homelessness or time in shelter. Only four families who had no available resources or support were sheltered and rapidly re-housed. Of the families Always Home diverted, 227 families (70% of total cases) were able to resolve their housing crisis with case management support only (zero financial assistance). The remaining 95 families received direct financial assistance, in addition to case management services, to sustain housing and avoid homelessness.
