Case Management Support
and Short-Term Housing Help
Case management is the primary tool used to successfully prevent a family from becoming homeless or entering emergency shelter. Every family receives one-on-one case management services including: individualized housing stabilization plans; budget counseling; landlord/family negotiations; basic tenancy skills; job coaching and search; connection to community resources; and, when necessary, short-term financial assistance to preserve existing housing situations or create new ones (security deposits, past due rents, rental subsidies, and utility arrearages.
In Fiscal Year 2019, Always Home served 232 client families seeking emergency housing assistance. By focusing on effective homelessness prevention strategies, 98.7% of our entire client caseload (229 families) was successfully diverted from shelter. Only three families who had no available resources or support were sheltered and rapidly re-housed. Of the families Always Home diverted, 143 families (62% of total diversion cases) were able to resolve their housing crisis with case management support only (zero financial assistance). The remaining 86 families received direct financial assistance, in addition to case management services, to sustain housing and avoid homelessness.
